Why choose us

Support and guidance you can count on

We make managing your support feel a lot simpler. Your main point of contact is your dedicated clinical Care Partner, a qualified nurse who takes the time to understand your goals and what matters to you. They guide your care every step of the way – looking after both the big picture and the finer details, while also coordinating your care team and connecting you with the right support for your needs.

Coordinated care that changes with you

Your care team work together as one, to ensure your services are coordinated and connected. With nursing, allied health and care management expertise working together around you, they keep track of your needs as they change, and provide support that’s right for you, now and tomorrow.

Staying in control of your care, made simple

Keep informed and in control of the support you receive, every step of the way, through our app. It’s where you can view your appointments, access your care plan, keep informed on spending, and get in touch with your care team – all in one place.

Choose who delivers your everyday support services

For cleaning, gardening, transport, meal preparation, social support and personal care, you can choose a provider from our network of trusted providers, or nominate your own preferred providers. Want to use a provider that’s not already part of our network? No worries. We support their onboarding process, while also ensuring they comply with required quality and safety standards. This gives you choice over who supports you, while knowing the right quality and safety checks are in place.

Whether you’ve been approved for Support at Home funding, are considering a change of provider or not quite sure where to start, we’ll help make the process easier to navigate, explain what to expect, and ensure you feel confident about what comes next.

Understanding costs and funding

Understanding your funding
Support at Home funding is based on your assessed care needs. Your budget is used to pay for approved services, and you may be required to contribute towards some services depending on your circumstances. Before services begin, we’ll walk you through any costs, contributions and fees so you know exactly what to expect and can make informed decisions with confidence.

What you’ll pay for
Clinical services, such as nursing and allied health, and personal care are fully funded by the Australian Government. For some independence and everyday living services, such as cleaning, gardening or transport, you may be asked to make a contribution. How much you contribute will depend on your financial circumstances and the services you receive.

Frequently asked questions

I am still unsure about what Support at Home is. Where can I find more information about the program?

Support at Home is an Australian Government program that helps older people access services to support their independence and wellbeing while living in their own home. It replaces previous programs like Home Care Packages and brings together a wider range of services under one system.

If you’re still unsure about what Support at Home is, you’re not alone. Here are some reliable places to get clear, up-to-date information:

How do I know if I am eligible?

To be eligible for government-funded Support at Home you must be over 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people).

An aged care assessment will determine if you are eligible for ongoing or short-term Support at Home services. If you meet the requirements, you can apply for an assessment straight away.

You can apply for an assessment via the My Aged Care website.

Can I switch from my current provider to Amplar Health?

Yes, you can. If you’re already receiving Support at Home services or currently have a Home Care Package, you can switch to Amplar Health.

We’ll work with you to make the change as simple as possible, guiding you through what’s needed and helping to coordinate your care along the way. We’ll take the time to understand your needs, bring your care plan together and help keep your existing support in place where possible.

Can I keep using my existing support workers or service providers?

In many cases, you can continue with the support workers or providers you already know and trust. For everyday services like cleaning, gardening, transport, social support and personal care, we’ll work with your chosen providers to get everything set up and make sure they meet the required quality and safety standards.

What’s the difference between self-managed and fully managed care?

Self-managed care gives you greater involvement in how your approved supports are arranged and delivered, including choosing service providers and having greater visibility over how your budget is used. Fully managed care means your provider organises and coordinates services on your behalf.

At Amplar Health, we support you through a self‑management approach. This means you have greater visibility and involvement in how your care is arranged, with the support of a dedicated Care Partner every step of the way. They’ll help you understand your funding, stay on budget, and find services that suit your needs, so you can feel confident making decisions about your care.

What is Amplar Health responsible for?

When you choose to self‑manage your Support at Home services, Amplar Health still has important responsibilities to keep you supported and compliant with government requirements. These responsibilities typically include:

  • Meeting all legal, clinical and safety obligations
  • Holding and administering your government funding
  • Ensuring your care services are safe and appropriate
  • Supporting you to develop and maintain your care plan
  • Providing advice when you need it
  • Managing incidents, complaints and emergencies
  • Ensuring continuity of care

How can I ensure my care will be safe and high quality?

Amplar Health helps ensure the services delivered to you are appropriate to your needs and meet required quality standards. We assess and onboard providers, monitor risks, support care planning and help resolve concerns if they arise. If you’re using your own providers, we’ll work with you to make sure the right safeguards are in place. If you ever have concerns, please reach out to our team.

How long does it take for me to start receiving services?

The time it takes to start receiving Support at Home services depends on a few key steps, including your assessment, your funding classification, and arranging a provider.

It can also depend on whether you have urgent needs, you’re accessing a short-term pathway, demand in your area, and whether you require complex clinical services or major home modifications.

How much will I have to contribute to my care?

What you pay under Support at Home depends on your individual income and assets, as assessed by My Aged Care. You only contribute to the non‑clinical parts of your support including things like transport, cleaning, gardening, and other everyday living or independence services.

However, it’s important to know that clinical care is fully covered by the Australian Government, no matter your financial circumstances.

To help you understand what your personal contribution might be for your non‑clinical supports, My Aged Care offers a fee estimator, which can give you an indication of your likely costs and help you plan your Support at Home budget. You can access the fee estimator anytime on the My Aged Care website.

How does care work with Amplar Health?

We help bring your care together, so you don’t have to manage everything on your own. Your dedicated clinical Care Partner, a qualified nurse, works with you to understand your funding, develop your care plan and coordinate your support.

When you need nursing, allied health or other clinical care, this can be provided by Amplar Health. For everyday services like cleaning, gardening, transport, social support and personal care, you have the flexibility to choose from our trusted provider network or nominate a provider you already know.

What can I do in the app?

The app helps you stay informed and keep track of your care in one place. You can view your care plan, see your upcoming visits, keep track of your budget and spending, and access your monthly statements. Once you select Amplar Home Health as your provider, we’ll send you everything you need to set up your account and access the app.

Can my family members be involved?

Yes, you can choose to involve family members or trusted people in your care. Through the app, they can stay informed, help you keep track of services and appointments, and support you in managing your care if you’d like. You’re always in control of who is involved and what they can see, so you can choose the level of support that feels right for you.

What happens if my needs change?

As your needs change, your support can change with you. You can update the mix of services you receive at any time, and if your care needs increase or become more complex, you can ask for a reassessment to access additional support. Your clinical Care Partner will work with you to review your plan, adjust your services, and support your wellbeing, comfortable, and independence at home.

What’s the difference between a registered provider and a service provider?

A registered provider helps oversee and coordinate your care under the Support at Home program, including managing funding and ensuring your services meet government requirements.

A service provider delivers the individual services you receive, such as cleaning, gardening, transport, nursing or allied health support.

In some cases, the same organisation may act as both a registered provider and a service provider. For example, Amplar Home Health is the registered provider responsible for coordinating and overseeing Support at Home services, while Amplar Allied Health may provide allied health services as an Associated Provider. Other services may be delivered by external service providers, referred to as Associated Providers by My Aged Care.

Can I choose my own registered provider?

Yes, you can choose the registered provider, such as Amplar Home Health, that best suits your needs, preferences, and budget.

It’s important to compare providers to find one that offers the right services and level of support for you.
