My Home Hospital

My Home Hospital is a SA Health service we deliver, providing acute, hospital-level care to patients in the comfort and privacy of their own homes. 

My Home Hospital has supported over 24,610 patients (as of 1 November 2025) in the greater Adelaide region since it commenced in January 2021. 

We continue to expand our scope to enable delivery of care to a wider range of patients, including post-surgical care options that address bed challenges for elective and non-elective surgery.

Transition Care Service

Transition Care Service is a joint collaboration between Amplar Health Home Hospital, SA Health and the Pullman Adelaide, providing an out-of-hospital health service for patients who no longer require acute hospital care.  

Through the Transition Care Service, we support a variety of patients, including: 

  • Patients who are medically stable but still require clinical care and other support whilst they wait for community or aged care accommodation, rehabilitation or hospital at home support to become available 
  • Post-operative patients who don’t require acute hospital care 
  • Those who require support while their carer is in hospital 

Patients are selected based on their clinical needs and are monitored by nurses and medical practitioners providing appropriate patient care before transitioning home or to an appropriate long-term facility such as an aged care home.

My Home Hospital

  • Since commencing in January 2021, the program has serviced over 24,610 (as of 1 November 2025) patients in the greater Adelaide region.  
  • Over 35+ doctors have been employed to deliver the service.  
  • The average length of stay in FY25 was 4.14 days. 

Transition Care Service

  • Since commencing in February 2025, the service has admitted over 422 (as of 1 November 2025) patients in the greater Adelaide region.  
  • The average length of stay is 14.8 days. 
  • Over 5,654 bed days have been provided at Transition Care Service rather than in the hospital.
  • Since opening in February 2025, the service has already expanded to an additional 47 beds.
