Support at Home

Providing care and support for senior Australians with more complex care needs.

The Support at Home program was introduced in November 2025, replacing the Home Care Packages Program and the Short-term Restorative Care Programme. It improves access to services, products, equipment, and home modifications to help older Australians remain living independently at home for longer. The Commonwealth Home Support Program (CHSP) will eventually transition to Support at Home, however this won’t be earlier than 2027.

How to access government funded support:

Support at Home may be an option if you need a coordinated approach to the delivery of your help at home including everyday tasks, or the care you need is more complex or intensive.  

To access government-subsidised aged care services, you will need to be registered with My Aged Care and undergo an assessment to find out if you are eligible for subsidised services. 

You can register and apply for an assessment by contacting My Aged Care on 1800 200 422 or through an online application.

Once assessed, Amplar Home Health will then guide you through the process and provide information on fees and costs associated with each program or service.

For some older Australians, having access to regular healthcare is an important part of staying healthy, connected and independent. Amplar Home Health is committed to supporting older Australians by providing services, where clinically appropriate, in the place they call home.
