Support at Home may be an option if you need a coordinated approach to the delivery of your help at home including everyday tasks, or the care you need is more complex or intensive.
To access government-subsidised aged care services, you will need to be registered with My Aged Care and undergo an assessment to find out if you are eligible for subsidised services.
You can register and apply for an assessment by contacting My Aged Care on 1800 200 422 or through an online application.
Once assessed, Amplar Home Health will then guide you through the process and provide information on fees and costs associated with each program or service.

Partnering with Amplar Health gives providers access to clinical programs purpose-built for the Support at Home environment. With a national network of allied health and nursing professionals, we work alongside home care providers to deliver care that’s coordinated, measurable, and genuinely person-centred from the first touch.
Every referral begins with our clinical assessment team, who review individual client needs, recommend the most appropriate pathway, and stay connected as a central point of contact throughout the program. From there, each pathway is supported by validated assessment
tools that measure progress, track wellbeing, and align with the Australian Government’s strengthened Aged Care Quality Standards.
