Registered providers choose to partner with us for:

  • Access to a national network of nursing and allied health expertise

  • Evidence-based assessment tools that strengthen clinical oversight and decision-making
  • Structured clinical services and pathways aligned with Support at Home requirements
  • Flexible referral options, from individual services through to broader partnership models

Our network of nurses, allied health professionals and care coordinators work alongside your team to extend clinical capacity and deliver clinical pathways and services within a robust clinical governance framework – which supports your organisation to increase capacity, help meet the Strengthened Aged Care Quality Standards, and support older people to maintain independence and wellbeing at home.

The right care from the start

Referrals are reviewed by our clinical team to ensure clients are matched to the most appropriate clinical service or care pathway based on their needs, goals and circumstances. We provide a single point of coordination, helping to streamline referrals, reduce complexity and deliver a consistent experience for both clients and providers. It’s how we support informed care planning and decision-making and build in greater visibility of the services being delivered and their impact on client budgets.

Expertise where it matters

Our network of nurses, physiotherapists, occupational therapists, podiatrists, dietitians and care coordinators work together to provide clinically-guided care that’s tailored to each person’s needs. By bringing together the right expertise at the right time, we help clients build confidence and maintain independence.

Flexible clinical services designed around client needs

From targeted clinical services to structured 12-week pathways, our services focus on early intervention, prevention and maintaining independence. By combining clinical expertise with a goal-focused approach, we help clients build strength, regain confidence and maintain capability, supporting better outcomes while helping make the most of available funding.

Visibility every step of the way

We provide regular updates throughout service delivery, including goal progress, clinical observations and any required escalations. For pathway participants, progress is reviewed throughout the program and supported by outcome measurement. In addition to regular progress notes, at completion providers receive a summary of outcomes and recommendations to inform ongoing care planning.

When to consider referring a client

Amplar Health supports older people with a broad range of clinical and functional needs. Our services may be suitable for clients who:

  • Require additional nursing or allied health support
  • Would benefit from restorative care following illness, injury or hospitalisation
  • Are at risk of falls, deconditioning or functional decline
  • Would benefit from support to improve their nutrition and wellbeing
  • Have changing needs that may benefit from specialist clinical assessment

Whether a client requires a targeted clinical service or a structured multidisciplinary pathway, our team works with you to identify appropriate support based on their goals, needs and available funding.

Trusted clinical care that’s shaped to each client’s needs

Our clinical care pathways bring together allied health, nursing and care coordination in structured 12-week programs designed to deliver measurable outcomes under Support at Home.

Focused on early intervention and personalised goals, each pathway supports older people to improve function, stay active and live well at home. Flexible pathway tiers adapt to individual needs, goals and available funding, while a robust clinical governance framework supports care that is coordinated, evidence-based and outcome-focused.

If a structured pathway isn’t the right fit for a client, we also offer individual nursing, physiotherapy, podiatry, occupational therapy and dietetics services, tailored to their specific needs. These can be accessed as standalone support services.

Restorative Care Pathway

Aligned with the Australian Government’s Support at Home Restorative Care Pathway, this pathway supports recovery following illness, injury or hospitalisation. Through allied health, nursing and coordinated care, this pathway helps clients improve function, mobility and wellbeing while supporting them to work towards meaningful goals and longer-term independence.

Falls Prevention Pathway

Combining physiotherapy and occupational therapy expertise, this pathway is designed to improve balance, mobility and safety to help reduce falls risk and support ongoing participation in everyday activities. Programs are tailored to individual needs, goals and circumstances.

Functional Strength Pathway

Through targeted exercise and allied health support, this pathway helps people maintain strength, mobility and physical function, supporting ongoing participation in everyday activities and helping them stay active for longer.

Nutritional Support Pathway

Led by dietitians, this pathway supports people experiencing nutritional risks such as poor appetite, unintended weight loss, or difficulties with eating and drinking. Through tailored assessment and support, it helps improve nutrition, energy levels and overall wellbeing.

From referral to service delivery, sooner

Getting started is simple. Complete the relevant online referral form and we’ll guide you through the rest.

The form adapts to the service or pathway selected, prompting you to upload any relevant documents along the way. This helps ensure we receive the right information upfront, reducing delays and helping clients access care sooner.

To help us build a complete picture of the client’s needs, goals and support requirements, we recommend including the Integrated Assessment Tool (IAT) and the client’s Support Plan with your referral. These documents help our team assess suitability, tailor recommendations and connect clients with the most appropriate services from the start.

Once we receive the referral, our clinical team will review the information and be in touch promptly to discuss next steps and coordinate the most appropriate pathway or service for your client.

Clear pricing for informed decision-making

Our pricing is transparent, with clinical care pathways structured across three tiers to suit different levels of client needs and funding.

Registered providers are billed only for services delivered, and formal quotes can be provided where required. Programs can also be tailored to individual goals, funding availability and budget requirements.

The Restorative Care Pathway aligns directly with Support at Home restorative care funding, helping providers make effective use of allocated funding while supporting improved function and meaningful outcomes.

Frequently asked questions

How do I make a referral for a client?

Complete the relevant referral form, which is available above. It is dynamic and will prompt you for any required document attachments based on the pathway or services selected. To support our team in understanding the client’s needs, we recommend you include the Integrated Assessment Tool (IAT) to determine an older person’s eligibility for aged care services and Support Plan.

What happens after I submit a referral?

Once a referral is received, our clinical team reviews the information and assesses the client’s needs, goals and eligibility for services. We may contact the referring provider for additional information if required. From there, we’ll coordinate assessment, make pathway recommendations where appropriate, and work with you to determine the most suitable approach for the client.

How do care partners receive updates about the service delivery?

The updates about the services are sent via our secure delivery platform to the nominated email address. After every completed visit, the handover and any assessment reports will be sent to the relevant care manager. This will keep the managers informed about progress, outcomes, or any concerns.

Does Amplar Health replace the provider’s care management responsibilities?

No. Registered providers remain responsible for overall care management and ongoing responsibilities under the Support at Home program. Amplar Health acts as an associated provider, delivering agreed clinical services and pathways while working collaboratively with the registered provider to keep care connected and coordinated. For Restorative Care Pathways, Amplar Health assumes the role of the Restorative Care Partner.

What is the difference between a clinical pathway and an individual service?

Individual services provide targeted support from a specific discipline, such as physiotherapy, nursing or dietetics, either as a one-off intervention or ongoing service. Clinical pathways take a more structured, multidisciplinary approach, bringing together the right expertise around agreed goals over a defined 12-week period. Pathways are designed to support measurable outcomes and a coordinated approach to care.

Who makes the final decision to proceed with the clinical pathway?

The registered provider/Care Partner in discussion with the client makes the final decision to accept or decline the recommendation. Our role when providing a service proposal is to assess suitability, provide evidence-based clinical recommendations, and support informed decision-making to provide the best care for the client.

What happens if a client’s Support at Home budget is limited?

We understand that managing budgets is a key priority. While we work within budget constraints, we will always seek approval for any proposed services and associated costs before commencing work.
