Knee Osteoarthritis

Better Knee, Better Me is a program for people with chronic knee pain from osteoarthritis, aiming to decrease the chances of needing total knee replacement surgery. Developed in partnership with leading researchers, this program is remotely delivered by physiotherapists, runs for up to 12 months and combines personalised care plans for exercise and pain management. Individuals with osteoarthritis who are above a healthy weight may experience more severe pressure on their knees, which is why this program may also include weight management support from a dietitian. Available to eligible Medibank members across Australia, Better Knee, Better Me makes high-quality osteoarthritis management accessible, wherever you are.

Type 2 Diabetes

Medibank’s Type 2 Diabetes Program is a chronic disease management service we deliver for those looking to achieve and maintain a healthier weight and manage their diabetes. This online program provides evidence-based support for managing type 2 diabetes, including personalised care for lifestyle changes, meal plan guidance and virtual consultations with dietitians. Having a chronic disease management plan for type 2 diabetes can help to decrease symptoms and manage blood sugar levels. The program runs for up to 12 months and is available to eligible Medibank members.

Cardiac Rehabilitation

Medibank’s Heart Health at Home is a remotely-delivered cardiac rehabilitation program we deliver to help reduce the risks of future complications and avoidable hospital readmission. The 4-6 week program includes weekly coaching calls with a dedicated clinician, meaning those who can’t access face-to-face cardiac rehabilitation can still get the care and support they need to improve their heart health. Individuals may receive support and counselling, a tailored program to work towards increasing physical activity, and guidance and advice through the Medibank Heart Health app. The program is available to eligible Medibank members, following an eligible cardiac event or procedure.

After Hospital Home Care

After Home Hospital Care (AHHC) is a trial service for those with chronic health conditions and specific accompanying risk factors to transition to and recover at home following a hospital admission. Our registered nurses visit patients in their home and then create tailored care plans to help support them in their ongoing recovery. The 30-day service is open to eligible Medibank customers and aims to decrease the likelihood of a patient needing to be readmitted back into hospital.

CareComplete

CareComplete is the suite of programs, comprising of CareFirst and CarePoint, delivered by Amplar Preventative Health (a business of Integrated Care Services Pty Ltd (ACN 059 950 695)). The programs provide individuals with, or at risk of, complex and chronic health conditions or injuries, the support and information they need to navigate the health system and manage their health, where clinically appropriate.

We focus on prevention

By focusing on prevention and helping people to manage their health, our CareComplete programs support individuals to try and stay well, which may help contribute to lower rates of preventable hospitalisations.

Our programs

Designed in consultation with participants, our chronic disease management programs take into account clinical suitability, personal goals and preferences for service delivery.

Care planning

Development and documentation of a chronic disease management plan with clearly stated health and wellbeing goals to work towards through the program.

Care coordination

Regular engagement with the individual to ensure they are receiving and accessing the right supports aligned to their individual needs.

Health coaching

Provision of lifestyle and general wellbeing support and education as a holistic approach to work towards the goals set out in their care plan.

Additional support

Assistance to link in with other services and supports that may assist health and wellbeing, including regular liaison with participants’ doctor.

Chronic disease management

Delivery of programs aimed at assisting individuals manage conditions such as heart disease, chronic lung disease, osteoarthritis and diabetes.

CarePoint

Amplar Preventative Health’s program, CarePoint provides integrated and coordinated support to help individuals navigate the healthcare system. Tailored to individuals with complex health needs, this program is designed to help manage long term health conditions and help people remain as well as possible. 

Through this program, individuals are assigned a dedicated care coordinator who will liaise with their doctor or other health care professionals, and where clinically appropriate, identify services based on their needs. 

CareFirst

Amplar Preventative Health’s program, CareFirst is a health coaching program designed for individuals living with a chronic health condition/s that can improve through behaviour and lifestyle changes. 

The program is delivered over the telephone or using a video platform and is designed to give individuals the confidence to manage their health. 

This program includes a personalised care plan, condition specific information, health coaching and support to access additional services.
