Amplar Preventative Health’s program, CarePoint provides tailored, personalised care plans for people with multiple long-term health conditions.

If you have complex, or multiple long-term health conditions, and have had frequent hospital admissions, managing your health can feel overwhelming. That’s where we can help. Our care coordinators can help you with general advice about your condition, to identify other services that may assist, and by liaising with your regular doctor to help coordinate your care.

What is CarePoint?

Managing multiple chronic health conditions and coordinating the different health professionals involved in your care can be extremely difficult.

Based upon international best practice, CarePoint coordinates a multidisciplinary team of qualified health providers to give you the support, skills and confidence to better manage your health and avoid unnecessary hospital stays. CarePoint is designed to help you get your health back on track, by giving you support to help you manage your condition at home.

Depending on your needs, the program can be delivered between for 4 to 12 months. A comprehensive care plan is developed with you and shared with your doctor, identifying a support network of health professionals and other care services, helping you get on top of your health concerns and stay on the path to good health.

Our tailored, personalised care plans aim to reduce the stress and confusion that often comes with managing a complex health condition. Your CarePoint program can also be adapted as your healthcare needs change, removing the guesswork of how you should be managing your health conditions. With a plan in place, we aim to leave you feeling more empowered and confident about managing your health.

How does CarePoint work?

CarePoint provides personalised coordinated care so you can easily access the care that you may need. Support services include:

  • a home assessment and tailored care plan
  • a dedicated care coordinator to help you implement health recommendations and access ongoing support.

How do I know if I’m eligible for the program?

You may be eligible for CarePoint if you have complex health needs such as diabetes, asthma or heart failure and/or you have had a recent hospital stay. To find out more about CarePoint or your eligibility please contact us.

Why is this program being offered?

Amplar Health wants to better support customers who are living with chronic and complex health conditions. We know it can be difficult to navigate the health system and access the care you need to stay well. We also know that if people stay well, it may reduce the pressure on the health system overall.

I’ve already got all these services and support in place, so why do I need CarePoint as well?

While CarePoint can identify additional services that may benefit you, one of the main aims is helping to better coordinate the services that you have access to. To the greatest extent that we can, we endeavour to make sure everyone is working together to support your health.

Care coordinators may also spend time helping you better understand and navigate the health system, with the goal for you to get the most benefit from your health care. Visiting you at home helps us better understand your circumstances, and we may identify other ways that we may be able to help, that you may not have considered. We can also support you to find help quickly if your circumstances change.

Is this program right for me?

The aim of CarePoint is to help you maintain control of your health and keep you doing the things you enjoy. What’s involved if I participate?

Once you’ve been invited to join the program and enrolled, a care coordinator will visit you in your home to undertake an assessment of your health care needs. This is designed to gather information about your current health state and any health and wellbeing goals you may have. Telephone support throughout the program may help you to improve your health self-management and access all the services you’re eligible for. The CarePoint care coordinator will support you, post-hospitalisation (should this occur) and identify if any other services can be put in place that may assist with reducing the risk of future hospitalisation.

What’s a care coordinator?

A care coordinator is a registered nurse or allied health professional such as a physiotherapist or occupational therapist. All CarePoint care coordinators hold current registration to practice with the Australian Health Practitioners Regulation Agency (AHPRA) or their relevant professional organisation. A care coordinator focuses on coordinating your care and assists you in identifying and supporting you to achieve your health goals.

What’s a customer care consultant?

A customer care consultant provides administrative support for the CareComplete programs. Customer care consultants will also contact participants to check in on their progress and provide information when required.

I’m feeling well now. I’m not sure how this program can help me?

It’s true that people on the program can feel well at points in time, but check-in calls are important to keep in touch.

Some people may not need a lot of support, so we may only speak to them every few months. However, sometimes we see or speak to people who are unwell every few weeks.

Your health can change unexpectedly. If you’re in the program and find that you become unwell, you can contact us and the team can then assist you.

Does this program cost me anything?

For eligible participants, the program itself is cost-free.

However, if there are costs involved for any of the services recommended during your program, and if you choose to go with those services, such as a dietitian appointment, physiotherapy appointment or other services, our care coordinator will discuss with you the options and associated costs beforehand. We will not enrol you into any program or for any additional services if you do not agree to them.

Depending on your level and types of private health insurance cover, your private health insurance provider may cover some of those costs. We recommend you contact your health fund to ask if they will cover the costs of those services and how much, before you elect to go with those services.

How is my privacy protected?

We are committed to protecting your privacy as part of your participation in the CarePoint program. We will handle your personal information in accordance with all applicable Commonwealth and State legislation, which governs the use collection, disclosure, storage, access and disposal of personal information.
